What is Hybrid Concierge Membership?
Concierge care is a style of medical care in which patients become members and receive enhanced medical services not normally covered by private insurance and/or Medicare. Our Hybrid Concierge Membership is different than traditional concierge care.
Hybrid Concierge has a much lower annual wellness investment than other traditional concierge programs.
It allows you to use your insurance and/or Medicare benefits for normally covered services at our practice. This helps to offset the expenses of a traditional concierge model, which does not bill private insurance or Medicare.
Our Hybrid Concierge Membership offers the most flexible, comprehensive, and personal care option for enhanced attention and services. With a Hybrid Concierge Membership, you can depend on Dr. Chan to be your personal physician, healthy living coach, and strong wellness advocate.

Membership Benefit Limitations, Policy, and Information
Dr. Chan's medical practice plan will be implemented each May 1 through April 30. Registration begins January 1 of the same year.
We provide an additional discount to the annual membership fee for couples and family members who also join.
To be a member of the Hybrid Concierge Program, you will be required to pay the annual membership fee regardless of benefit usage. We will bill your insurance and/or Medicare for covered medical services and you will be responsible for your copay.
Membership is not a replacement for insurance and/or Medicare. Your insurance and/or Medicare will be billed for normally covered services rendered by Dr. Chan and his staff. Additionally, outside of Dr. Chan’s office, you will still need medical insurance for emergency room visits, hospitalization, specialists, prescriptions and all services provided outside of the office.
Due to the nature of the Hybrid Concierge practice, we devote more time and attention to each patient who joins. Membership is limited and offered to patients on a first-come, first-serve basis. This way, we ensure each patient who becomes a member will receive the highest level of attention and care possible. Priority is given to established patients.
Although we do our best to make this plan affordable, we realize there will be some of you who will be unable to afford membership. If that is the case, I welcome you to remain a patient of my practice, unlike a traditional Concierge model. Your care will be primarily managed by our qualified resident Nurse Practitioners under my supervision. Should you require hospitalization then your care will be managed by my appointed hospital affiliated colleagues.
There may be times when we agree that an emergency room visit is necessary. You are responsible for charges incurred for services not provided by Dr. Chan. If admission to the hospital is necessary then Dr. Chan will coordinate care for you with the hospital-based specialists.
Fees charged for prescriptions are not covered by membership fees or benefits.
If you must transfer your care, we will assist you in locating a new physician. Your medical records will be sent promptly upon your release of information. The balance of your yearly fee will be prorated and refunded. Money may be withheld depending on the extent of services received prior to cancellation of your membership.
